Dr. Bawa accepts insurance with out-of-network benefits. Our billing department can check if your insurance has out-of-network benefits. Please fill out the insurance verification information including full name of patient, patient's DOB, Insurance name, insurance ID and the name of primary insurer on insurance. Please also provide your phone number, and we will call you after insurance verification to discuss your benefits and schedule the evaluation visit
